1. Application in writing address to Chairman Welfare Services Committee SSWA Karachi, duly signed by applicant with Address, Contact number, Copy of CNIC

2. Application should be recommended by any one of member of Executive Committee or any 2 Life members (List of Executive Committee and Life Members is available on SSWA Website)

3. Application should be delivered in Head office of SSWA located at Mazzanine Floor Plot. 9.C, Sunset Lane.6 Phase.II Extension DHA Karachi, Mr. Waseem Office Secretary (03337113816)
